The FA and Birmingham County FA aims to use the internet and e-mail to benefit as many Referees as possible, whilst not detracting from a system that remains in place for those not wishing to use IT as a form of communication.
A range of misconduct forms are available on-line. Choose from the documents in the related links section (right) to download.
Once downloaded and completed, Referees should save and re-name the document* before attaching it to your e-mail message.
The form(s) should then be sent to the relevant e-mail address:

Sending Off/Akin Offences ONLY - discipline@birminghamfa.com
Only The FA's approved Report Forms sent to the correct e-mail address will be accepted. Personally designed electronic forms will not be processed.
*File > Save As > select where the on your computer the file will be saved in the drop down menu at the top of the pop up window.
Enter a unique file name (eg HOMEvAWAYTEAM+DATE) in the relevant box at the bottom of the window. Click the "Save" button on the bottom right of the pop up window.
